Our practice has gone green! When you schedule your new patient consultation you will receive an email with a link to open your New Patient Paperwork. Please fill out and submit the paperwork 48 hours prior to your appointment. This time has been reserved exclusively for you. Please notify us at least 24-hours in advance if you are unable to keep this appointment by calling 770-351-7737 and press option 2.
